The following are general guidelines established in the Blanket Purchase Agreement:
• College orders may be placed by fax, phone, blanket purchase order (BPO), or VISA P-Card. (Note: The supplier is currently developing an online order form and you will be notified upon completion)
• Personalization of names on shirts shall be department names only, (i.e., Purchasing, Payroll, Finance, etc.) Individual employee names are not allowed on any shirt
• No single purchase transaction shall exceed $2,499.99
• Delivery: Approximately 14 calendar days after receipt of order (unless a garment is back ordered)
• All orders include free shipping and desktop delivery service
• No minimal order quantity
• Pricing formula:
Total cost of garment: A + B = C
A. Garment: Suggested retail x 50% (Example: 19.98 x 50% = $9.99) plus
B. Logo embroidery = $4.00 plus
C. Subtotal cost of $9.99 + $4.00 = $13.99. (Divide subtotal cost of $13.99 by 77% = College net total cost or $18.16 on a $19.98 retail garment)
Other: Red House labeled garments = 60% discount (retail x 60% discount . . .)
• Additional pricing:
1. Verbiage (i.e., Purchasing) under College logo = $.60 per department/program name
2. Verbiage or website on yoke, collar, cuff, or right chest, etc., = $2.50 (up to 4,000 stitches)

Note: College employees may independently purchase garments from FSP, receive Blanket Purchase Agreement College discounts, and pay by using their personal debit/credit card or pay by cash.

All software requests should go through the same process:
1. Faculty should refer to the list of site-licensed and individually-licensed software to verify that their request is not currently available Collegewide and if their software is already loaded in an available classroom on their campus. If the software is site-licensed, faculty must log a helpdesk ticket and their campus technician will load the software on their machine(s).
2. If the software is not on the site-licensed list or on their campus, faculty must then complete an Academic Software Request Form with the assistance of a campus technician and the approval of their Dean, Campus DAS, and the Campus President before being submitted to Ms. Lillie Hookfin in the Information Technology Department.
3. Please be aware that completion and approval of the software by Campus Leadership does not guarantee software acquisition. Software purchases will be dependent on available funds. Certain departmental software requests may require additional committee or functional dean approvals (i.e., Foreign Language software).
4. Software requests should match textbook adoption. Please verify the software recommended for your course based on your textbook for the entire academic year.

Now there’s an easy way to change your voicemail password and settings via the web. The Cisco Personal Communications Assistant allows users to change passwords, set playback speed, change greetings, etc.
To reset voicemail passwords:
1. Open a browser such as Internet Explorer or Safari.
2. Type: ‘ciscopca’ in the address bar. Brings up a page: “Cisco Personal Communications Assistant”. The full address is http://ciscopca.fccj.org/ciscopca
3. If prompted, install any ActiveX plugins.
4. Login with your userid, password, and ‘fccj’ in the domain field.
5. Click the link ‘Cisco Unity Assistant’. This should bring you to the Personal Preferences page.
6. Fill in the fields about half way down the page for ‘New Password’.
7. Make any other changes on the page.
8. Click ‘Save’. Changes should be immediate.
*Note: Phone passwords will soon be set to change every 60 days. You can’t use the same password within a year’s time, or any of the 4 previous passwords.
